Testimonial Tribute

Hello Grace members and friends,

We are asking those in our congregation to write a testimonial tribute to someone who was instrumental in developing their foundation of faith. At the conclusion of our campaign, we will create a book of all of the tributes as a reminder of those who made an eternal difference in our lives.

You may print the form (or pick one up at the welcome tables) and hand-write your tribute, or follow the steps below to type right on the form:

1) Download the form and save it on your computer.

2) Open the document using Adobe Acrobat Reader

software (you will need to download this safe, commonly-used software first if you don’t already have it).

3) Click on the purple “Fill and Sign” icon in the

toolbar along the right side of the page.

4) Complete the form and save it.

Once you have completed the form, please return it in one of the following ways:

Email it to cyndi@gracepclv.org
Drop it off at the church office
Mail it to the church at 1515 W. Charleston Blvd, Las Vegas, NV 89145

Now that we are in the Thanksgiving and Christmas season, we encourage you to take time to reflect and write a tribute in honor or in memory of someone who blessed you by sharing their faith. Happy Thanksgiving!
